Care Coordinator

Person Centred Pathways is looking for an enthusiastic and focused Care Coordinator. We need someone who is very well organised and works well with a team as a whole. This is a full-time position, and it requires the employee to manage a lot of different aspects of care coordination. The Care Coordinator will have oversight of not just the visits and overall care that we provide, but also the schedule that our providers must adhere to. The person who fills this role must be an excellent and effective communicator, as they will have to interact with family members, healthcare professionals, and other providers of care. Additionally, the Care Coordinator will participate in our on-call schedule, which means that the person filling the role must have a valid driver's license and reliable transportation.

Main Duties:

  • Handle efficient client visitation and scheduling.
  • Maintain and update staff rosters for clients.
  • Internationalise with families, healthcare providers, and other staff members.
  • Be available and ready to give support if it is needed.
  • Be part of the on-call system, ready to handle urgent situations and staff attendance problems if they arise.

We are looking for candidates with the following qualifications:

  • Good at math and can read, write, and do computer work fairly well.
  • Must have a positive attitude, show initiative, and be willing to learn.
  • Should have experience in nursing care or being a receptionist.
  • Absolutely must be an excellent communicator; this position demands it.
  • Should be capable of working independent from others and be self-motivated.
  • Must have strong organisational skills and the ability to handle emergencies.
  • Should be able to speak, with good etiquette, on the phone and in face-to-face meetings.
