Our Approach to Care: The Person-Centred ( Care Model)
At Person Centred Pathways, we are committed to offer sympathetic and individualised help that matches the singular requirements of every person we serve. We use the Person-cantered Care model, which makes the person receiving care our main focus. This means that we organise our care around the desires, values, and autonomy of the individual.
STEP 1: Contact our Local Office
You can start your journey to excellent home care by contacting our local office. We understand that you may prefer different forms of communication, so we offer several ways for you to get in touch. Whether you want to talk on the phone, send an email, or have a sit-down meeting, we are here for you and we'll continue to be here every step of the way, working toward providing the right home care for you and your family.
STEP 2: Assessment of Needs
After you select Person Centred Pathways as your care provider, we will organise a meeting to assess your care needs. During this session, we will meet with you (or your loved one) along with other important people in your life, such as family members and caregivers. We will work together to understand your care requirements. The outcome will be a Care Plan that reflects what we know about you and the outcome of those meetings—tailored specifically to your life and your unique set of circumstances.
STEP 3: Care Plan and Other Documentation
After completing the assessment, and working together with you, we will create a Care Plan that is truly person-centred, taking into account all of the specifics unique to the person's situation. This plan will be your opportunity as the family member, or as an advocate for the person, to outline all of the services that you think would be needed and to articulate, also, how you think those services should be delivered. When all of those details are decided, our Care Team will have the task of generating a specified quotation and contract for you. The quotation will convey the costs associated with the services to be provided. Once you're satisfied with the documents, we'll finalise the contract, and service can commence.
STEP 4: Service Starts
After finishing the essential paperwork, the care services will get going by following the Care Plan we have designed. Our compassionate and committed caregivers, chosen for their ability to connect and understand what it means to provide care, will visit the person in need and apply their personal touch and skills in a way that respects that person’s individuality and independence. Those same qualities of professional presence and the art of communication are replicated in our welcome folder of information that we leave with the family or the person served ensuring transparency and accountability throughout the care process.
STEP 5: Ongoing Care
Ongoing care is provided with periodic reviews to ensure that all your needs are being well cared for, and you are satisfied with the service we are providing. We will conduct Care Plan Reviews with you and your family (and any other key people) to discuss any changes that have occurred and to make sure that everything is still going according to plan.
