We're looking for a Care Manager to join our team—to do vitally important work in the lives of disabled adults and the elderly. If you have an inner drive to manage folks who, due to age or physical condition, can't get through the day without someone else doing some or most of it for them, then we'd like to hear from you.

Key Responsibilities:

  • Supervising a support workforce that takes care of individual clients, guaranteeing they receive the personalised attention they need.
  • Scheduling healthcare workers to pay regular visits to their assigned clients and planning the specifics of the care to be provided.
  • Designing a care plan that is as unique as the person for whom it is being created, tailoring it to their needs and wants.
  • Managing work schedules and ensuring that our program has the right number of workers, available at the right time, to make all of our individual and family commitments work.
  • Needing to use our knowledge and understanding of dementia to grasp the situation each family faces and creatively solve problems alongside them.
  • Supporting the wonderful program supervisors and program staff who work tirelessly to provide a safe and nurturing environment for our clients.
  • Ensuring the proper documentation of clients’ progress, including the recording of medications they receive and any incidents or concerns that happen in the program.

Requirements:

  • The ideal candidate is someone who has worked in this type of job before. We are looking for reliable and responsible people.
  • Full-time availability is required and, if necessary, the ability to work overnight.
  • Preferably, our candidates would have a valid driver's license.
  • Good people and communication skills are necessary for this job.
  • NVQ/RQF Level 3 or equivalent qualification preferred.
